Policies & Procedures
Before We Get Started
Once you schedule an appointment, you will be sent over electronic documents that should be filled out at least 24 hours before your scheduled appointment. The documents consent of intake information needed to secure your appointment. If your documents are not completed, you may be asked to reschedule at a later time.
All information will be reviewed, limiting the need to fill out any paperwork at the time of your appointment. You are asked to bring your driver's license, insurance card, and any other documentation requested for your chart.
Payments are collected at the start time of each session.
